The operator shall furnish the city secretary at the time of the original application
and as new drivers are employed by the operator, the following information on each
driver: age, height, weight, chauffeur's license, address, training and experience
in transportation of passengers. The operator shall also sign a statement to be furnished
to the city secretary that after making a careful investigation he is of the opinion
that the driver has a current chauffeur's license, is physically fit to chauffeur
passengers, is not addicted to drugs or intoxicating liquors so as to render him unfit
to perform the duties of a driver, that he is a person of good moral character, and
that he has not been convicted of a felony or a misdemeanor other than a traffic offense,
or if convicted of a felony offense, the date, nature and disposition of such offense.
There shall also be attached a recent photograph of the driver.
